We would like to invite you to participate in the HASC Workplace Wellness Survey. This survey is designed to collect information from health care organizations across the state on the strategies, design, objectives, incentives, measurement, evaluation, and communications they are using in their workplace wellness programs.

Survey data collection began on January 6, 2014, and runs for approximately four weeks.

The survey should be completed by a human resources director, manager, or other administrator who has direct knowledge of the health care organization’s employee wellness initiatives, participation, health plans, incentives, and outcomes.

Report highlights will be presented at the HASC Health Care Provider Wellness Conference to be held on June 12-13, 2014 in Anaheim. Participants who complete the survey by the deadline will be entered into a raffle at this conference to win a ZipTM Wireless Activity Tracker, the latest fitness device from Fitbit to track steps, distance, and calories on your computer, to help you meet your personal activity goals.
