Registration is currently open for Improving Patient Care Transitions to Reduce Readmissions – A Multifaceted Approach, a webinar scheduled for Thursday, June 20, 11:30 a.m. to 1 p.m.
Ideal for chief medical officers/physician leaders; chief nursing officers/nurse leaders; hospitalists; case managers and discharge planners, the session will be led by Dr. Eric Coleman, director of the Colorado Care Transitions Program, and will describe a multifaceted approach that has proven to be effective and include the key steps to operationalize true patient-centered care along the continuum.
Participants will:
- Identify the central goal of transitional care
- Describe the challenges hospitals face in reducing readmissions
- Identify a seven‐pronged approach to improving quality and safety during care transitions
- Learn the key steps to operationalize true patient‐centered care
- Learn what the future holds with respect to new initiatives aimed at reducing hospital readmissions
The session is sponsored by HASC and the Hospital Council of Northern California. Program cost is $200 for HASC members, $275 for associate members and $350 for nonmembers.
