HASC’s 3rd Annual Palliative Care Conference, Post Acute Palliative Care Continnuum: Connecting the Dots, will take place on Wednesday, Feb. 9 at the Pacific Palms Hotel & Conference Center in the City of Industry.
This one-day program moves the focus of palliative care’s rapid evolution to the post-hospital continuum of care. Representatives from physician groups, skilled nursing facilities, hospices and home care agencies are also invited to the event.
Topics will include: Moving Upstream: Palliative Care Outpatient Practices; How an Integrated System of Care Connects the Dots - Healthcare Partners Model; The CareMore Palliative Care Continuum; Palliative Medicine Resource Hotline: PAL-MED CONNCECT; and Best Practices Panel / Story Board Discussion.
