HASC recently hosted a webinar on Whole Person Care – Los Angeles to review the eligibility criteria and referral process used by hospitals for individuals who qualify for the program. (The event was broadcast on Sept. 29.)

The pilot program’s purpose is to ensure that the most vulnerable Los Angeles County residents have the resources and support they need to cope with adverse situations. Referral services include Homeless Care Support Services, Justice-Involved High-Risk, Mental Health High-Risk, Perinatal High-Risk, SUD High-Risk and Medical High-Risk.

Whole Person Care brings together health and social service delivery entities across the County, building an infrastructure to deliver seamless and coordinated services to Medi-Cal beneficiaries. Qualifying individuals include those with behavioral health and other challenges that lead them to frequent utilization of EDs and other facilities.
