Person-Centered Care Project Seeks Partners to Explore Initiative Benefits
HASC, in collaboration with the California Hospital Association and the Hospital Quality Institute, is leading a Person-Centered Care Initiative, funded by The SCAN Foundation. The goal of this three-year initiative is to assist provider communities (hospitals and their primary care, post-acute, and community-based organization partners) to deliver person-centered care throughout the continuum. Such support is especially critical for older adults with chronic conditions and functional limitations.
Person-centered care strives to treat each patient as an individual. For example, following these principles, care providers ask each patient what is important to him or her — and then seek to implement those personal goals over the course of care. Person-centered care also seeks to respect patient preferences as conditions change, which could include loss of mental or physical functioning or changes in care settings.

HASC will select 3-4 leading-edge provider communities who will collaborate to explore and implement person-centered care models, coordinate existing community partners and develop a business case for program sustainability.
