Person-Centered Care Report Released
HASC recently completed a meaningful study on person-centered care. Successful strategies and lessons learned are shared in a report published by the association.
A new care model for patients – especially seniors – person-centered care asks individuals to define their own treatment goals, which then guide care for months and years to come. Recently, multiple evaluations demonstrate that the model significantly improves quality of care, patient satisfaction and cost effectiveness – both in and out of the hospital.
HASC worked with Adventist Health White Memorial, Community Memorial Health System, Eisenhower Health, and Keck Medicine of USC to run four parallel case studies on person-centered care in action. Each is summarized in the report.
Guided by a strategic planning group and aided by a collaboration of community-based organizations, physician advocates, workshop facilitators, and model developers, the project provided valuable insights on integrating concepts into practice. The strategic plan calls for technical assistance and peer-to-peer learning over a three-year period – from April 2016 to March 2019.
As explained in the report, the project underlines “the importance of collaboration: between patient and provider within the hospital, and between hospitals and community organizations.”
