Nomination forms for the 6th Annual Hospital Hero Awards, scheduled for Friday, Nov. 11, 2011, at the Marriott Hotel, Downtown Los Angeles, are now available at www.hasc.org/2011HospitalHeroAwards.

The awards are intended to highlight the exceptional achievements of health care professionals in member hospitals throughout the HASC region. All hospital staff members who have made an outstanding contribution to the hospital’s unique service delivery mission within the past 12 months are eligible, including medical, technical, clinical, administrative and volunteer.
