Thank you 2013 Sponsors!

About the Event

Start the conversation with a new partner in the care continuum at the 5th Annual Palliative Care Transitions Conference. The conference strives to break down barriers inhibiting optimal care delivery at all points in the care continuum.  Attendees gain information about pioneering efforts at expanded physician-to-physician conversations as well as health plan perspectives on achieving  “triple-aim” payer-provider reimbursement alignment. Enhance your knowledge and share your team skills with others guiding the development of new service delivery models in supportive care.

  • Hear from physicians about their personal transformations and how better listening has improved the quality of their practice
  • Learn about productive physician/physician conversations in patient management
  • Understand the critical tipping point of choosing between “comfort care” and general inpatient care (GIP)
  • Recognize how the term “supportive care” is more helpful in obtaining advance care planning goals

Plus panel presentations and group networking opportunities will present best practices and concrete strategies from your peers highlighting:

  • The pharmacist’s contribution
  • How a home health provider achieved a 4% hospital readmission rate
  • The patient/family advocate role
  • Re-energizing your team
  • Obtaining executive buy-in

Who should attend?

We are inviting physician groups, home health and hospice providers, hospitals, health plans, and skilled nursing facilities to attend.  This includes physicians, administrators, nurses, nurse practitioners, social workers, pharmacists, chaplains, and consultants representing key stakeholders in a coordinated, integrated continuum of providers needed to increase the awareness of the economic and quality of life benefits supportive care options provide for chronically ill patients and their families.
