About the Event
Start the conversation with a new partner in the care continuum at the 5th Annual Palliative Care Transitions Conference. The conference strives to break down barriers inhibiting optimal care delivery at all points in the care continuum. Attendees gain information about pioneering efforts at expanded physician-to-physician conversations as well as health plan perspectives on achieving “triple-aim” payer-provider reimbursement alignment. Keynote Speakers
The speakers and tentative presentation topics for this year’s event include:
Robert Bear, MD - How a Nephrologist Became a Palliative Care Medicine Champion through Personal Epiphany and Change Leadership
Dr. Bear practiced as a nephrologist (kidney specialist) for over 30 years. He was a professor of medicine at the University of Toronto and subsequently at the University of Alberta. He also held senior management positions in several health care organizations, worked broadly as a health care consultant, and published widely in the medical sphere.
He will be signing his book Sorrow’s Reward.
Richard Brumley, MD, Chief Medical Officer, GeriNet - Patient Dilemma: Aggressive Care vs. Supportive Care – My Wishes, and Physician to Physician: Approaching the Reluctant Physician
Dr. Brumley has worked on numerous grants and projects to further the specialty of Palliative Care. He was the head of the Hospice and Palliative Care teams at Kaiser Permanente before joining HealthEssentials as Chief Medical Officer. HealthEssentials serves the frail elderly through the GeriNet Medical Group, home health, and hospices. He has written numerous published articles on palliative care and end-of-life care.
Pat Forman, Consultant, The Schwartz Center for Compassionate Healthcare – Schwartz Rounds
Pat is a health care consultant offering advance care planning trainings and workshops for organizations; organizational consulting to health care agencies and organizations; and life coaching for individuals seeking to make positive changes in their lives.
Mark Livingston, PharmD, CareRx – The Optimizing Role of the Pharmacist
Mark is a clinical pharmacist with an extensive background in clinical and consulting pharmacy, including hospital pharmacy management, long-term care consulting, and clinical advisory services to health care professionals. He is responsible for CareRx operations involving clinical advisory service and for consulting with clients regarding compliance with Federal and State regulations.
Mark Leenay, MS, MD, National Medical Director & Senior Vice President, OptumHealth – Post-Acute Transitions in Patients with Complex and Chronic Conditions
Dr. Leenay is responsible for clinical policy in wellness, case management, disease management and specialty networks. Board certified in family medicine, geriatrics, and hospice and palliative care, he previously served as director of palliative care for a large system and served 10 years in family practice.
Glen Komatsu, MD, Director, Edmond R. and Virginia G. Doak Center for Palliative Care, Little Company of Mary Hospital – Torrance - Comfort Care vs. General Inpatient Care (GIP) … Who’s Getting G.I.P.’d
Dr. Komatsu became medical director of the Neonatal Intensive Care Unit for Little Company of Mary Hospital in the South Bay area in 1985. With an interest in bioethics and palliative care, he has also served as the director of Bioethics, Little Company of Mary Service Area since 1993.
