HASC’s Person-Centered Care Initiative is looking for 3-4 provider communities to explore and implement the care model in their organizations. Also in this issue of HASC Briefs, the association congratulates Dan McLaughlin and Julie Miller-Phipps on new executive positions, and National Health Foundation’s Hospital Hero Awards take place at the Skirball Cultural Center on Friday, Nov. 4.

Person-Centered Care Project Seeks Partners to Explore Initiative Benefits

HASC, in collaboration with the California Hospital Association and the Hospital Quality Institute, is leading a Person-Centered Care Initiative, funded by The SCAN Foundation. The goal of this three-year initiative is to assist provider communities (hospitals and their primary care, post-acute, and community-based organization partners) to deliver person-centered care throughout the continuum.  Such support is especially critical for older adults with chronic conditions and functional limitations.
