The Bridges Prenatal to Three Network (Bridges Network) supports
children’s success by identifying health and developmental
concerns during the critical first years of life, and by
providing families with education, screening, and linkage to
services including referrals for home visitation services by
public health nurses and other professional staff.
The Mental Health Hospital Triage Services (MHHTS) program is a
collaboration of Orange County hospitals and the County of Orange
Health Care Agency-Behavioral Health Services department, working
together to ensure patients presenting in the emergency
department have the support and resources needed for their
behavioral health recovery.
HASC announces a business partnership with Alegis Revenue Group,
LLC.
Alegis, which specializes in revenue recovery on accounts that
require heightened expertise, labor and capital including third
party eligibility, third party liability, and account resolution
and appeals, manages more than $3 billion in charges for more
than 240 hospitals nationwide. It has helped clients meet their
cash goals by creating additional revenue that would have
otherwise gone unrealized.
The Bridges Maternal Child Health Network (Bridges Network),
funded by the Children and Families Commission of Orange County
(Commission) through Proposition 10 tobacco tax revenue, is a
multi-disciplinary system of providers that collaborate
throughout Orange County to improve the health and well-being of
pregnant women and young children 0-5 years of age. The Bridges
Network was redesigned and rebranded in 2009. The aim was to
restructure the program to operate as a network system of care;
identify cost savings and reductions; find ways to streamline
processes; and strengthen program management. All redesign
recommendations were based on emerging best practices and
evidence-based models of care.
Although Orange County’s new program for the remaining uninsured
population commenced on January 1, it will not be until late this
week that hospitals will be able to start taking applications to
cover services provided to those with urgent or emergent medical
conditions. Patients in the new Medical Safety Net (MSN) program
are required to access services through hospital emergency rooms
only, even if they were served in the county’s prior program and
suffer with chronic, debilitating conditions that have been
treated in other, more appropriate settings such as community
clinics.

Rapid Applications for Medical Programs (RAMP) is a customized
web-based tool used to screen for financial assistance programs
like Medi-Cal, CMSP, Crime Victim Compensation, Section 1011 and
hospital assistance/charity.
First introduced to HASC members last year, RAMP is already
providing a significant return. Centinela Hospital Medical Center
began RAMP implementation March 15, 2012. Within 60 days, the
hospital screened 170 patients.
There is still time to participate in HASC’s Case Management
Transformation Initiative (CMTI), scheduled to begin in October.
Powered by Marsh, CMTI is an interactive nine-month,
results-oriented transformation program that will provide your
hospital with the tools to transform your current case management
function into a comprehensive, physician-centric, outcomes-based
program.
The Bridges Maternal Child Health Network (MCHN), under the
management of Orangewood Children’s Foundation (OCF) and funding
from the Children and Families Commission of Orange County
(CFCOC), provides a multi-disciplinary system of care to improve
the health and well-being of pregnant women and young children
0-5 years of age throughout Orange County. MCHN recently released
its Fiscal Year 2011-12 report.

What is RAMP?
RAMP, short for Rapid Applications for Medical Programs, is a customized web-based tool to screen for financial assistance programs in your state, counties and hospitals, including:
Medi-Cal Programs for the Aged, Blind, and Disabled, as well as Family programs for Parents, Pregnant Women, Minor Children under 21, Breast and Cervical Cancer Treatment Program, etc.
CMSP: County Medical Services Program Serving rural counties in California
Crime Victim Compensation
Section 1011 program for undocumented aliens
Hospital Financial Assistance/Charity Program
During the interview process, RAMP identifies the best potential program for your uninsured patient based on household and financial considerations. It then provides the appropriate application form for the program, auto-populating certain fields and allowing staff to include attachments like medical releases and charity documents. The system even facilitates income estimation, address verifications and the gathering of other financial information so your staff can save time, achieve greater accuracy—and faster reimbursements.

Working with patients along the continuum of care is increasingly
common. When your patients cannot care for themselves and need
help managing their finances or other personal needs after
discharge, the Conservatorship Access Network (CAN) offers
comprehensive probate conservatorship evaluation at your
facility.
The Eligibility on Site (EOS) program provides participating
facilities with on-site Medi-Cal certification on a full- or
part-time basis. Eligible patients can be interviewed and
enrolled on site, either before treatment or after discharge,
cutting red tape and securing appropriate reimbursements much
more quickly.

Allocate your hospital resources more efficiently. HASC’s
partnership with the California Children’s Services (CCS)
provides participating facilities with on-site eligibility
technicians to help facilitate the CCS screening and enrollment
process for your patients.
